ABSTRACT

Background: Emergence of extended spectrum ?-lactamase (ESBL) producing strains of Gram-negative bacteria can lead to serious infections frequently complicates the clinical and treatment outcome. Aims and Objectives: The purpose of this study was to know the prevalence of ESBLs and to know the most common Gram-negative bacteria, which produce ESBLs at our health-care facility. Materials and Methods: This study comprised all of the isolates of Gram-negative bacteria that were acquired from various clinical samples. For the purpose of the investigation, a sample of an isolate that showed resistance to two or more third-generation cephalosporins was taken. ESBL detection and antibiotic sensitivity tests were carried out using conventional microbiological techniques. Results: We isolated a total of 284 Gram-negative bacteria and 54 (19%) were identified as ESBL producers. Out 54 ESBL producers, 18 (33%) isolates were Escherichia coli, 11 (20%) were Pseudomonas aeruginosa, 12 (22%) were Klebsiellae, 6 (11%) were Enterobacter, 2 (4%) were Citrobacter, 4 (8%) were Acinetobacter, and 1 (2%) were Serratia spp. Conclusion: Clinical decision-makers can make the best antibiotic treatment by regularly monitoring multi-drug resistant bacteria like ESBL producers. This also helps to improve infection control procedures. In addition, we must maintain reserve medications like carbapenems on hand for judicial use.

ABSTRACT

Objective To analyze the overall situation of regular monitoring of occupational hazards in enterprises in Guangdong Province in 2022. Methods Analysis and quality sampling from enterprises were conducted on the occupational hazard regular monitoring reports submitted by various occupational health institutions through the Guangdong Province Occupational Health Quality Control Platform for the year 2022. Results In 2022, a total of 40 129 enterprises in Guangdong Province conducted regular monitoring of occupational hazards, accounting for 5.9%. More than 4.85 million workers were enrolled, of which more than 1.99 million workers were exposed to occupational hazards, with an incidence of 41.1%. The incidence of exposure to chemicals, dust, and noise were 18.2%, 11.9%, and 23.5%, respectively. The incidence of exceeding national standard for chemical substances, dust and noise in enterprises were 2.7%, 3.3%, and 42.7%, respectively. The incidence of exceeding national standard for all occupational hazards increased with the scale of enterprises from micro, small, medium to large enterprises (29.1% vs 46.7% vs 61.3% vs 65.4%, all P<0.05). The top three key industries, with more enterprises exceeding national standard, were metal furniture manufacturing, wooden furniture manufacturing, and other metal daily necessities manufacturing. The concentration of 97 chemical hazardous agents such as silica dust and benzene in work site exceeded the national standard, but less than 1.0% chemical hazardous agents exceeded national standard in most of the chemical-exposed work site. The incidence of noise exceeding national standard was 45.4%, while the incidence of silica dust exceeding national standard was 17.3%. Conclusion The percentage of regular monitoring of occupational hazards in enterprises in Guangdong Province is relatively low. The main occupational hazard that exceeds the national standard was noise, indicating the need for special attention on noise protection in workplace.

@#Abstract: Objective To analyze the occupational hazards of enterprises in Pingshan district of Shenzhen in 2017. Methods Occupational hazards were analyzed in 200 enterprises in Pingshan district of Shenzhen City selected using stratified Results random sampling method. A total of 24 industries were involved in the 200 enterprises. The declaration rate of , occupational hazards was 91.5% and the exposure rate of occupational hazards among workers was 49.2%. The regular monitoring rate of occupational hazard factors in workplaces of the enterprises was 79.5%. There were 129 kinds of occupational , , hazard factors of which 19 factors exceeded the national occupational exposure limit accounting for 14.7%. The over standard , , , , , , , , rates of noise silica dust cotton dust methanol toluene and other dust were 28.7% 13.6% 11.8% 5.86% 0.5% and , , 0.4% respectively. There were 13 kinds of occupational hazard factors in the workplace of metal products industry all of which ( ) exceeded the occupational exposure limit. The exposure rate 56.7% of occupational hazard factors in workers was the highest. Conclusion , , The main occupational hazard factors were noise dust and chemical factor and the major occupational hazard industry was metal manufacturing in Pingshan district of Shenzhen City.

ABSTRACT

Objective To observe regular monitoring in patients with chronic myeloid leukemia (CML) who received tyrosine kinase inhibitor (TKI), and to analyze its influencing factors. Methods A total of 857 patients with CML in Henan Tumor Hospital from October 2012 to October 2016 were collected. Patients were told to receive regular monitoring after receiving TKI treatment, including blood routine, bone marrow, BCR-ABL fusion gene and chromosomes. All patients were divided into good and poor compliance groups according to regular monitoring. Chi-square test was used to compare ABL kinase domain mutations rate and mortality between two groups. TKI species, level of education, duration from diagnosis to treatment, teaching times, sites of follow-up, convenience of transportation, annual income and gender were recorded respectively, and the factors affecting regular monitoring were analyzed by using single and multiple factor analysis. Results There were 390 and 467 patients in good and poor compliance groups respectively. Treatment failure rate was 19.49% (76/390) and 25.91% (121/467) in good and poor compliance groups respectively, the mutation rate was 28.95% (22/76) and 7.44% (9/121) respectively. The difference of ABL kinase domain mutation in patients with treatment failure of both groups was statistically significant (χ 2 =16.287, P < 0.01). The mortality was 0.77% (3/390) in good compliance group, and 2.78% (13/467) in poor compliance group, and the difference was statistically significant (χ 2 = 4.543, P = 0.033). The single factors analysis showed that TKI species, level of education, duration from diagnosis to treatment, teaching times, sites of follow-up, convenience of traffic and annual income were related with regular monitoring (all P < 0.05). Multiple-factor analysis showed that inconvenient transportation (β = 1.56, 95% CI 1.74-3.74, P = 0.014), low education level (β = 1.67, 95% CI 0.81-3.12, P = 0.041) and low income (β = 2.87, 95% CI 1.31-4.51, 95%CI 1.74-3.74, P = 0.011) were independent factors for poor compliance in regular monitoring. In the result detection, 56 fusion genes fluctuated. Conclusions CML patients who received regular monitoring have a low treatment failure rate and mortality. Inconvenient transportation, low education level and low outcome are independent risk factors for regular monitoring. The single monitoring result can not prompt treatment effect, and thus it needs to review and monitor for many times.
